  • Your name, address and phone number
  • Your policy number
  • The date and time your loss happened
  • A brief description of what happened

Claim Process - Life Business

Step 1

Report claim through phone call, e-mail, letters or claim center.

  • Your name, address and phone number
  • The insurance policy number (Natural/Accident)
  • Nature of claim request- Death, Disability Claim, Medical Expenses, Surrender/Loan, Full/Partial maturity

Step 2

Check Basic Documents required for various heads of claims

(a)    Requirements for Death Claim

  • Original Policy document
  • Proof of Identity of the Claimant
  • Completion of the Claimant form (CL-1) by the beneficiary
  • Completion of the Physician’s Statement form (CL-2) by the Medical Doctor who certified the Assured dead
  • Completion of Identification Statement form ( CL-3) by a disinterested third party who is cognizant of the death of the Assured
  • Birth Certificate of the Minor beneficiaries
  • Marriage Certificate
  • Medical Certificate of Cause of death
  • Autopsy/post mortem Report (for coroner cases)
  • Police Report for Accidental cases
  • Completed Release form (CL-15)

(b)    Disability Claim

  • Completion of Accident/Sickness questionnaires
  • Completion of Claimant Statement form CL-105
  • Completion of Continuous disability form  CL-106
  • Completion of Employers Statement form CL-107
  • Completed Release form  CL-5

(C)    Full Maturity

  • Original policy document. But where Original policy document has been lost, form PS-1
  • (Loss of policy form) has to be completed
  • Completed Release form (CL-15)
  • Proof of Identity

(D)    Partial Maturity

  • Completion of the computer generated notice
  • Proof of identity-e.g. Extract from int’l passport, I.D. Card

(E)    Requirement for Loan

  • Original policy document
  • Proof of Identity
  • Completion of form Ps 13 (Loan Agreement Form)

(F)     Requirement for Surrender

  • Original Policy document
  • Proof of Identity
  • Completion of form PS 30 (Surrender form)

Download and fill claim form

Attach necessary documents (Basic Requirements)

Send your claim papers.

Note:

(a)    Once your claim is filed, the documents will be reviewed by our claim officers and you will be contacted within one business day.
(b)    After a fair assessment, your claim will be completed as quickly as possible and for valid claim, the due benefit will be will be released to the claimant within the next 48 hours.
